Atrial Fibrillation Detection
Challenge
Managing patients pre and post AF Ablation procedure often requires them to attend regular clinic appointments to check the progress of their condition. For patients this is not only stressful and worrying, it can also be inconvenient and difficult to fit in with daily life. Responding to ad-hoc patient enquiries takes clinical time. If patients are doing well in treatment pre and post AF ablation, a scheduled clinic appointment may not be necessary.
Solution
Benefits
• Reduce unnecessary face-face appointments, saving clinician time
• Reassure patients that they are being cared for
• Empower patients to take control of their condition and manage their health
• Save patient travel time and costs
Cardioversion
Challenge
Managing patients pre and post Cardioversion procedure often requires them to attend regular clinic appointments to check the progress of their condition. For patients this is not only stressful and worrying, it can also be inconvenient and difficult to fit in with daily life. Responding to ad-hoc patient enquiries takes clinical time. If patients are doing well, a scheduled clinic appointment may not be necessary.
Solution
Benefits
• Reassure patients they are being cared for
• Empower patients to take control of their condition and manage their health
• Patient outcomes better with improved medication adherence
• Reduce unnecessary clinic appointments
Undiagnosed Palpitations
Challenge
Diagnosing an individual’s cardiac arrhythmias can be like finding a needle in a haystack. Cardiac episodes often happen outside the time of prescribed monitors typically worn for 24 hours up to 30 days.
Solution
Benefits
• No specialist medical devices to buy or manage
• Convenient and easy for patients to use
• Patients feel reassured
• Reduce unnecessary clinic appointments
Atrial Fibrillation Medical Management
Challenge
To manage patients on medication for Atrial Fibrillation often requires them to attend regular clinic appointments to check for arrhythmia. This can be inconvenient and difficult to fit in with daily life and takes up clinician time and appointment slots.
Solution
Cardiovascular Disease Risk Screening
Challenge
Cardiovascular disease is the leading cause of premature mortality and morbidity worldwide. Screening programmes can identify at-risk patients early to undergo interventions that will improve their outcomes and reduce the downstream health service costs of managing their severe disease. However, screening programmes can be costly and inconvenient for healthcare services to provide and often require patients to visit a clinic and undertake tests that require clinical experience and use specialist medical devices.
Benefits
• Reassure patients that they are being cared for
• Empower patients to take control of their condition and manage their health
• Save patient travel time and costs
• Enable clinicians to remotely monitor and titrate medication
• Reduce face to face appointments, saving clinician time
Solution
Benefits
• Easy and convenient for patients
• Improve outcomes for at-risk patients, identified earlier for preventative advice and treatment
• Reduced face to face appointments saving clinician time
• Reduce downstream healthcare costs of costly medication, A&E visits, hospitalisation etc
Heart Failure
Challenge
People recently diagnosed with heart failure are often required to attend many appointments. This requires them to juggle weekly hospital appointments and restricts daily life. It also results in a substantial number of face-to-face appointments, home visits and clinical time.
Solution
Benefits
• Reduce face-face appointments saving clinician time
• Reassure patients that they are being cared for
• Empower patients to take control of their condition and manage their health
• Save patient travel time and costs
• Enable clinicians to remotely monitor and titrate medication
Post Stroke Pathway
Challenge
Recovery from a stroke can be prolonged and will continue at home and in the community. Providing patients with support and the feeling that they are being well cared for whilst at home is complex and resource intensive for care teams as well as often requiring patients to attend many clinic appointments.
Solution
Benefits
• Improve patient’s feeling that they are being properly supported and cared for whilst continuing their recovery at home
• Empower patients to take control of their condition and manage their health
• Enable clinicians to remotely monitor patients and intervene quickly for appropriate advice and treatment
• Avoid cost and management of specialist devices
